How to Create a Person Centred Nursing Care Plan

There should be a detailed Care Plan not just for any medical issues the person has, but there should also be a detailed Care Plan for general day to day living activities such as sleeping, nutrition, and recreation or pastimes and the individual’s preferences for each of these. 

A Care Plan should be so detailed that an agency worker or new carer could, by just reading the Care Plan, be able to deliver the care required by that individual, which is specific, person centred, and agreed.


Consultation

It is helpful to have a definition of the medical issue to enable the nurse, carer, person who needs the care and his or her relative, to understand fully what that issue or condition is.

The person creating the Care Plan must consult with the person and his or her relative, to find out their understanding of the medical issue and what concerns they have regarding it. ey have regarding it.
 

Assessment

The person creating the Care Plan should make an assessment of the particular medical issues faced.

Care Planning

This sets out clearly and in precise detail the planned action to address the medical issues and the daily living activities of the individual.
