Colostomy Nursing Care Plan

A colostomy is a surgical procedure to divert one end of the large intestine, the colon, through an opening in the abdominal wall, the stomach. The end of the bowel is called a stoma. A pouch is placed over the stoma to collect waste products that usually pass through the colon and out of the body through the rectum and anus, back passage. This provides a new path for waste material and gas to leave the body. A colostomy can be permanent or temporary.

When the colon, rectum, or anus is unable to function normally because of disease or injury, or needs to rest from normal function, the body must have another way to eliminate the waste. A colostomy may be used to treat the conditions such as, bowel cancer, Crohn’s disease, Diverticulitis, Ulcerative colitis, and anal cancer.

The Care Plan sets out a clear explanation of the resident’s issue, and will guide the nurse or carer through the process of preparing a comprehensive, individual person centred Care Plan.
