Dehydration Nursing Care Plan

Dehydration occurs when the amount of water leaving the body is greater than the amount being taken in. To test for dehydration, take the skin on the forearm or forehead using the index finger and thumb and squeeze gently. If the resident is dehydrated the skin takes time to return to its normal position.
