The successful discharge of any patient from hospital relies on effective joint working between NHS, social care partners and the independent sector. Sharing information about investigations which have been carried out, any  treatment received, and reviewing the patients care is good practice. This will improve the discharge and transfer out of hospital experience for the patient and improve the planning of future care.
